Terry Lee Polm, 49, of Chillicothe, died April 3, 2009 in the Ohio State University Hospitals, following a long battle with cancer. He was born August 22, 1959 in Marion, IN to John and Freda D. Stevens, who preceded him in death. Calling hours will not be observed. Following cremation, graveside services will be held.
